Detection of MALT1 gene rearrangements by FISH using AP12 and MALT specific probes in the peripheral blood lymphocytes of the Helicobacter Pylori seropositive patients

  • Lymphoma of gastric-mucosa-associated lymphatic tissue (MALT) type has been linked to Helicobacter Pylori(Hp) infection.
    Purpose: The peripheral blood lymphocytes(PBL)ofH.p seropositive patients carry MALT1 gene rearrangements.
    Methods: Fifty-one pts suffering from chronic iron deficiency coexisting with megaloblastic anemia,12 leukopenic,2 with thrombocytopenia,3 with B-lymphocytosis,1 with undefined leukocytosis,3 pts with polycythemia Vera and 9pts with a low value of IgG globulin are included in the study.
    The control group was:1pt suffering from gastric Ca,1pt with a gastric MALT lymphoma,2pts with a malt lymphoma of the parotid gland,1pt had a malt lymphoma of the mouth and 4 children coming from them.All had a high title of IgG /IgA anti-H.p antibody in serum, H.p infection on endoscopic biopsy,and H.p-associated gastritis.All received H.p eradication therapy, and 4 were resistant to it.
    PBL were cultured using standard techniques.Thirty GTG banded metaphases were analyzed (ISCN1995).
    FISH was done in 40pts using theLSI MALT1(18q21)dual color probe,LSI IGH/MALT1t(14;18)(q32;q21),LSI AP12/MALT t(11;18)(q21;q21)probe set (VYSIS).
    Results: The karyotypes were normal.With FISH,23/40pts had a simple
    rearrangement of MALT1 gene.Five pts had trisomy 18,17pts had as a partner in the translocation #14, 4 pts had #11 and 2pts had both chromosomes.
    Conclusions: PBL-MALT1 gene rearrangements can be a very early event of a forth-coming disease in H.p seropositive pts.
    The prevalence of t(14;18)/t 11;18) and the spectrum of the arising disease are not cut clear.So criteria must be done in endoscopic biopsies specimens to recognize clinically relevant tumor progression in time.
    The pts with PBL-MALT1 gene rearrangements can develop a MALT gastric lymphoma as a response to chronic H.p infection. Prospective and retrospective investigation must be done and thereby help to further define precisely the clinical and pathological characteristics of H.p infection.
